Each row is one SMTP account. The worker rotates round-robin; per-row limit caps sends per run (0 = unlimited). Used when a campaign's gateway is smtp.

Direct-to-MX delivery

No relay. The worker resolves each recipient's MX and speaks SMTP directly. Requires an open outbound port 25, a forward-confirmed PTR, and SPF/DKIM/DMARC.
